We are required to collect and forward the applicable DOJ and/or FBI fees for each application if the application does not have a billing number. The Requesting Agencies’ FORM BCII 8016, “Request for Live Scan Service” ORI NUMBER, LEVEL OF SERVICES, & BILLING NUMBER (if applicable) will determine the exact services and fees required. State fees are usually found on the instructions of the Request for Live Scan form or can be looked up through the licensing website.
